Acceptance requirements

1. A public place sanitation permit has been obtained; and 2. the application for extension is made at least 30 days prior to expiration of the validity period of the permit.

1. Application

(1) The applicant shall visit the service page of Shenzhen Municipal Health and Family Planning Commission at the website of “Guangdong Province Administrative Service” (http://www.gdzwfw.gov.cn/portal/branch-hall?orgCode=693956469), where an account should be created at the first visit, select the relevant service, and make an online appointment for application. If in need of document delivery by express, the applicant shall also provide the mailing information of the recipient at the time of the online application.

(2) After the online appointment for application is made, the applicant shall submit all required materials in hard copy to the Administrative Service Hall in a timely manner. After receiving the applicant’s application for administrative examination and approval, the Administrative Service Hall shall, if the examination and acceptance decision cannot be made on the spot, record the application and issue to the applicant a written acknowledgement of receipt of the application materials, otherwise the application shall be moved to the next step.

2. Acceptance

(1) Acceptance examination: The implementing authority shall examine whether the application materials submitted by the applicant are correct and complete in accordance with the standards of form and catalog within two working days of receipt thereof.

(2) Materials supplementation or correction: In case of application materials being incomplete or not in line with statutory forms and therefore requiring supplementation or correction by the applicant, the handling person shall notify the applicant of such supplementation or correction all at once within two working days. The applicant shall submit the supplemented or corrected materials within 30 working days, otherwise the application shall be dealt with as having been returned.

(3) Acceptance decision: The implementing authority shall accept the application if the acceptance requirements are satisfied, or if the application is not accepted, issue and serve a decision on rejection.

3. Examination

(1) Means of examination: Documentary examination and field examination combined shall be completed by the implementing authority within five working days.

(2) Documentary examination: The examining personnel shall conduct documentary examination within two working days after acceptance of the application for administrative examination and approval, mainly examining if the application satisfies the statutory requirements and standards. If all such requirements are satisfied, the examining personnel shall render a conclusion of satisfaction. If no requirement or only some of such requirements are satisfied, the examining personnel shall render a conclusion of no satisfaction, together with specific reasons, and notify the applicant of the conclusion.

(3) Field examination: Within three working days after the documentary examination is passed, at least two examining personnel shall be designated by the implementing authority to carry out a field examination before preparing an onsite note or, if rectification is required, sanitation supervision opinions. The applicant shall complete the rectification work within 40 working days (the time for rectification shall not be included in the time promised for handling), and submit a rectification report in writing to the Administrative Service Hall.

4. Approval

Based on the conclusion of examination made by the examining personnel, the decision-making person shall decide whether to approve or disapprove the application within two working days.

5. Conclusion

The staff shall prepare the examination and approval document within one working day after the decision is made.

6. Service

The applicant may obtain the result by express delivery, in-person claiming, authorized claiming or other means. The original of the receipt of materials shall be presented to claim the application result. In case of loss of the original of the receipt of materials, the valid identity document of the person in charge of the medical institution (original required for verification) shall be presented. For authorized application, the letter of authorization and the valid identity document of the authorized person (original required for verification) shall be presented, in addition to the aforesaid materials.
