Acceptance requirements

A medical institution having obtained a medical institution practice license shall apply for alteration of registration to the original registration authority in case of alteration of any of the following practice information: (1) address of practice; (2) type; (3) class; (4) clinical item(s); (5) number of beds (dental chairs, dialysis beds); (6) service recipients; (7) name; or (8) the person in charge of the medical institution.

Processing procedures

Online processing procedure

1. Application

(1) The applicant shall visit the website of “Guangdong Province Administrative Service” (http://www.gdzwfw.gov.cn/portal/branch-hall?orgCode=693956469), select the relevant service, and make an online appointment for application. If in need of document delivery by express, the applicant shall also provide the mailing information of the recipient at the time of the online application.

(2) After the online appointment for application is made, the applicant shall submit all required materials in hard copy to the Administrative Service Hall in a timely manner. After receiving the applicant’s application for administrative examination and approval, the Administrative Service Hall shall, if the examination and acceptance decision cannot be made on the spot, record the application and issue to the applicant a written acknowledgement of receipt of the application materials, otherwise the application shall be moved to the next step.

2. Acceptance (2 working days)

(1) Acceptance examination: The implementing authority shall examine whether the application materials submitted by the applicant fit the formal requirements in accordance with the standards of form and catalog within two working days of receipt thereof.

(2) Materials supplementation or correction: In case of application materials being incomplete or not in line with statutory forms and therefore requiring supplementation or correction by the applicant, the handling person shall notify the applicant of such supplementation or correction all at once within two working days. The applicant shall submit the supplemented or corrected materials within 40 working days, otherwise the implementing authority is entitled to make a decision on rejection.

(3) Acceptance decision: The implementing authority shall accept the application and issue a decision on acceptance if the acceptance requirements are satisfied, or if the application is not accepted, issue and serve a decision on rejection.

3. Examination (15 working days)

After acceptance, the competent health and family planning authority shall render a preliminary opinion. Two staff members shall be designated or experts shall be appointed to carry out a field examination before preparing an onsite note or expert onsite appraisal opinions. If experts propose rectification, the competent health and family planning authority shall issue a notice of rectification. The applicant shall carry out rectification work in accordance with relevant requirements, and submit a rectification report in writing to the competent health and family planning authority after completion thereof. The examination shall be completed within 15 working days, including three for documentary examination and 12 for field verification, and excluding the time for special procedures, i.e. field examination of 10 working days and rectification.

4. Approval (5 working days)

The responsible person of the competent authority shall decide whether to grant approval based on the examination opinion and materials, following which a medical institution practice license shall be issued upon approval, or a decision on rejection of administrative approval upon disapproval.

5. Conclusion (2 working days)

The staff shall prepare the examination and approval result with the official seal affixed.

6. Service

The examination and approval system will automatically send a text message to notify the applicant of the examination and approval result and of claiming a decision on rejection of administrative approval or medical institution practice license. The applicant is required to bring along all application materials that have been submitted online to the service hall to claim the result. The applicant may obtain the same by express delivery, in-person claiming, authorized claiming or other means. The receipt of materials or the original of any other acknowledgement shall be presented to claim the application result.

In case of loss of the receipt of materials or the original of any other acknowledgement, the resident ID card or other valid identity document of the person in charge of the medical institution (original required for verification) shall be presented.

For authorized application, the letter of authorization and the resident ID card or other valid identity document of the authorized person (original required for verification) shall be presented, in addition to the aforesaid materials.
